The isovolumetric contraction as marker of ventricular performance in patients with afterload mismatch

چکیده
Abstract Introduction The gold standard for the evaluation of ventricular function is currently echocardiographic measured ejection fraction (EF). However, use this parameter has limitations, which are more important in populations with afterload mismatch and especially when severe. On other hand, assessment a pump fluid dynamics done using head pressure, product pressure flow zero. During cardiac cycle, situation occurs during isovolumetric contraction. Purpose aim study to assess clinical pathophysiological importance isovolumentric contraction performance. Method 110 patients severe aortic stenosis pulmonary arterial hypertension were investigated. area under curve (AUC) was curves right ventricle-pulmonary artery left ventricle-aorta ascendens. This AUC then correlated (EF), stroke volume (SV) total work, defined as flow. Results correlates statistically significant corresponding ventricle (p 0.0001). Both showed statistic significance work (AUC R2 0.49, p 0.0001, EF 0.51, Stroke only correlation fraction. A one sample t test could be found (decreased, 0.001) (increased, but not ventricle. Conclusion mismatch. Especially these it demonstrates increase compared normal values. Funding Acknowledgement Type funding sources: None.

• "Afterload," defined as the external factors that oppose the shortening of muscle fibers, is as important a determinant of myocardial performance in the intact heart (1-3) as it is in the papillary muscle preparation (4). Many investigators are searching for an "index" of contractile state that will be independent of all external forces, but until there is more general agreement on such an in...

It has been previously shown that after acute coronary occlusion, the extent of systolic bulging is dependent on preload and the function of the remote nonischemic myocardium is influenced by the motion of the ischemic myocardium as well as by the loading conditions. To examine the isolated effects ofchanging afterload on the movement of acutely ischemic and nonischemic myocardium, seven open-c...
